Alex's Lemonade Stand Foundation for Childhood Cancer

Financial aid provided by Alex’s Lemonade Stand Foundation is made possible by a generous grant from TD Bank. Currently, these funds are available to pediatric cancer patients that travel into, or out of, one of the following medical institutions for childhood cancer treatment:

• Children’s Hospital of Philadelphia (PA)
• Children’s Hospital of San Francisco (CA)
• Children’s Hospital of Seattle (WA)
• Children’s Mercy Hospital (Kansas City, MO)
• Children’s Memorial Hospital (Chicago, IL)
• Cincinnati Children’s Hospital Medical Center (OH)
• Connecticut Children’s Medical Center (Hartford, CT)
• Dana-Farber Cancer Institute (Boston, MA)
• Doernbecher Children’s Hospital (Portland, OR)
• Hospital for Sick Children (Toronto, Canada)
• Johns Hopkins Hospital (Baltimore, MD)
• Kosair Children’s Hospital (Louisville, KY)
• MD Anderson Cancer Center (Houston, TX)

• Texas Children’s Cancer Center (Houston, TX)
• Vanderbilt University Medical Center (Nashville, TN)

In addition to the above, the following criteria should be met:

1. The patient is 18 years old or younger.
2. The patient must be diagnosed with cancer.
3. The patient’s annual family income should not be greater than $75,000.
4. The patient must currently be receiving treatment for childhood cancer.

Please follow these guidelines while applying:

1. Existing resources should already be maximized for each family.  This program is meant to be a supplement, and is intended to only fund additional out of pocket travel and accommodation expenses for the families.
2. Maximum allotment per family per year should not exceed $2,000.
3. Assistance is not retroactive. ALSF will not allocate any funds until we receive all the information.
4. Patients may travel to a facility or from one facility to another institution as long as the other guidelines are met.
   

If the patient is eligible, he or she may receive assistance with the following expenses:

• Travel: Airline tickets and gas cards
• Lodging: Hotels & philanthropic housing
• Meals: Food vouchers from the hospital’s on-site cafeteria and local area restaurants

We will accept applications through certified representatives (i.e. social worker, nurse, doctor, resource specialist, etc), who will apply on the family’s behalf. All sections of the form must be completed truthfully. Any false or misleading information will result in an automatic denial.

After Submission:

• An ALSF representative will review the application. We understand that time is of the essence for many children and their families and will do our best to respond to all requests within 1-2 business days.
• ALSF reserves the right to change these guidelines at any time without notice, and to apply these guidelines at its reasonable discretion.
